Preface
We will be profiling and featuring all projects that are submitted to the Matching Grant challenge. If you have already submitted your project into the competition, it qualifies for profiling and promotion. Enter early, even if your whole project proposal is not completed. You can request to have your project put into update/edit mode until the submission deadline on June 15, 2017.

Sample Grant Application Form
This is a PDF showing the grant application form itself. You will need to fill it out online and upload the requested files by going to the submittal portal. Until then, you can use this form to see what will be expected and what information you will want to gather. We suggest you download this .pdf as a reference. When you are ready to submit, you will go to the submittal portal and you will set up an account and start your submission process.
Sample Authorization Letter
Following is a sample for the kind of letter you will need to get for your project Memorial’s controlling entity or entities.
The first page one is a sort of “formula” for a suggested letter. Following that is an example of how that letter would read for a fictitious project being done by a Boy Scout troop in Springfield Illinois.
Your authorization letter would, of course, reflect the details and conditions of your project. We provide this reference as a support resource for your efforts.
Project Plan Outline
The objective of the Project Plan section of the submission is simply to affirm that you have your ducks in a row. Specifically, that means the jury reviewing your proposal simply wants to make sure you have made a plan around a realistic assessment for what you are getting into and how to accomplish your idea.
If a group is undertaking an effort like this for the first time, the plan will help think through the elements of the project. If you are experience in implementing projects, making this type of plan is a natural part of the process.
So this is less of an “exam paper” than it is a checklist (for you and us) that you have thought through the elements and the process you are undertaking.
